ROOT CAUSE ANALYSIS MODEL
The Honeycomb is an analysis tool we use to better describe and understand the details surrounding an incident.
The tool can also be used to understand how we might approach a task or job, identify risk and hazards, and implement a safety-minded decision making process.
ENVIRONMENT
Environmental factors aren’t limited to the weather and ecosystem. Things like crowds, time of day, and the attitude of others can all affect an environment. Be aware of external conditions that can increase risk.
CONFLICTING VALUES
In the quest to provide an experience of a lifetime often conflicting values may be encountered. For instance, rushing to serve a guest may compromise Safety. Look for these situations and be aware of the potential risks and pressures.
POLICIES & PROCEDURES
Policies and procedures are intended to keep people safe, but that doesn’t mean they are all perfect. If something is out of date, incomplete, or just plain wrong, work with the team to make it right.
TACTICAL CHOICES
Deciding which trail to take at the end of an exhausting day is a tactical choice. So is choosing not to wear approved footwear. When it comes to choices, remember to ask “is this the safe thing to do?”
TOOLS, KNOWLEDGE, & SKILLS
To do a job right, people need to be adequately trained. But sometimes there may be gaps in skills or knowledge. Periodically review trainings and expectations. Speak up if something is missing.
INSTITUTIONAL PRACTICES
Institutional practices are "the way things are done around here." They are usually accepted practices but may not always align with a safety culture. Keep a critical eye on what is being done day-to-day and make sure it doesn’t conflict with Safety.
WELLNESS
How might emotion, fatigue, scheduling, or workload impact our ability?
Are you adequately nourished, hydrated, rested, and properly dressed for the conditions?
Are there ergonomic, job rotation, or repetitive motion concerns with the workplace?
